The ISS office will have representatives from the Social Security Office here on campus to help you apply for your social security number. You must have current employment or a signed job offer to apply for a social security number.

Come to the Holcombe Hall Living Room on Wednesday, Sept. 13th any time between 1:00-4:00pm. No appointment needed.

Please bring the following documents with you:

•    An original Employment Verification Letter printed on departmental letter head with an original signature in ink.

(Download Employment Verification form and email to employer to fill out and print on University or UA department letterhead. Signature must be signed with ink pen.)

•    J1 Exchange Visitors (students) need an Employment Authorization Letter from your program sponsor.

•    Completed and signed Social Security Number (SS5-form). Complete the form online at www.ssa.gov/ssnumber. Print out online application reference number to bring to appointment.

•     Signed I-20 (page 1 and 2) or DS-2019 (page 1) – Original and a photocopy

•      U.S. Visa – Original and a photocopy (exception are Canadian citizens)

•      I-94 (found at www.cbp.gov/i94) – Original and a photocopy

•      Passport – Original and a photocopy of the front cover of the passport AND the ID Page with your name, date of birth and passport expiration
